How to write a Witness Statement
A witness statement refers to a method of evaluating a learner’s knowledge, skills, behaviours, or performance by gathering information and feedback from individuals who have observed the learner in a specific situation or context.
Witnesses are individuals who are in a position to provide credible and relevant information about the learner’s performance. These could include line managers, supervisors, mentors, colleagues, or others who have directly observed the learner’s actions.
The witness observes the learner as they perform the task, skill, or activity and take note of the learner’s actions, decisions, and behaviours.
A Witness Statement is a report on an event that demonstrates application and competency of knowledge, skills, and behaviours in line with the apprenticeship standard.
In your witness statement you will provide a brief overview of:

Detail type of activity witnessed.

What did you witness, where, when, etc?

How does this meet the requirement of the standard?
All witness statements must be supported by work product dated evidence, such as a context statement and screenshots, to prove the activities took place. Your witness statement will be void if there is no supporting context statement from the learner and work product dated evidence provided.
